The foundation is a separate philanthropic entity linked by shared history and governance through the Sheppard Pratt Health System. The foundation holds charitable assets, while Sheppard Pratt Investment manages the health system's institutional endowment, with legal separation between the two pools.

While the originating mission is behavioral health, the investment strategy extends to general healthcare services and Maryland-centric real estate. The Concord Health Partners partnership channels capital toward health-care innovation broadly, and the direct real estate portfolio includes mixed-use and industrial assets not tied to clinical operations.
